Make a Breast Thermography Appointment

Note: If you are enrolled in an Employers Resource Consumer Direct, Preferred or Deluxe Medical Plan, your thermographic scan may be considered an eligible wellness expense (up to $200 a year). Here's how to proceed with getting your thermographic scan paid for by Employers Resource:

1. Consult the Wellness Section of your Employers Resource Medical Plan.

• Consumer Direct Plan ("Health Care Provider Services" Section, "Routine and Preventative Services" paragraph)
• Preferred Plan ("Health Care Provider Services" Section, "Preventive Wellness Care Benefit" paragraph)
• Deluxe Plan ("Health Care Provider Services" Section, "Preventive Wellness Care Benefit" paragraph)

2. Based on the criteria in your plan, schedule an appointment with a Thermography Clinic (a number of clinics are listed immediately above).

3. After your appointment, have the clinic submit the claim per the instructions on the back of your Medical ID Card. If the clinic wants you to pay up front, then do so, then send the receipt and a completed claim form (accessible here) to P5 Health Plan Solutions, PO Box 9554, Salt Lake City, UT 84109-0554.
